Ubuntu 16.04 LTS boot menu dissapear after several use of computer

After installing ubuntu in my machine (fully installed and no windows os currently residing in the disk).
At first when installation is done, ubuntu would normally load and is working fine that i could see the boot menu order.
boot priority #1 ubuntu
boot priority #2 SATA DISK
But after resting the computer or after several reboot ubuntu boot menu wont up show anymore and i always got this message
Reboot and Select proper Boot device
or Insert Boot Media in selected Boot device
I followed the instruction here to get boot-info and here is the output: http://paste.ubuntu.com/p/S4xdSWCBrb/
When i try to format my computer again and reinstall ubuntu the menu stated that ubuntu 16.04 LTS currently exist in the disk and ask me to erase and reinstall again or install ubuntu 16.04 LTS alongside ubuntu 16.04 LTS.
I also notice that whenever it happens the bios menu configurations are resetting.
I need some help.
