Here is a picture of my sound output devices on Ubuntu 17.10 on Gnome.
Sometimes there are devices shown there, but frequently there are not. How to I get it so my headphones or built-in laptop speakers always show up?

try looking in Pulseaudio Volume Control configuration tabpavucontrol -t 5
. Devices may be detected in Pulseaudio but set to 'Off'
